Our Commitment

High Focus Centers follows the requirements of the Health Insurance Portability and Accountability Act (HIPAA) and applicable state law. Protected health information (PHI) is used for treatment, payment, and healthcare operations, and disclosed only with your authorization or as permitted or required by law.

How We Use and Share PHI

  • Payment. Submitting claims and verifying benefits with your insurer.
  • Healthcare operations. Quality review, training, accreditation, and care coordination.
  • As required by law. Reporting required by public-health, safety, or judicial authority.

Your Rights

  • Inspect and request copies of your record.
  • Request corrections to your information.
  • Request restrictions on certain uses and disclosures.
  • Receive an accounting of certain disclosures.
  • Receive confidential communications by alternative means.
  • File a complaint without retaliation.
